Fit timing belt in following order: Crankshaft sprocket. Water pump sprocket. Camshaft sprocket. Tensioner pulley. NOTE: Observe direction of rotation marks on belt. Fit tensioning tool locating pegs to tensioner pulley holes -Tool No.1.860.745.100/300. Ensure tensioning tool arm is horizontal. Important note on timing procedure: The correct way to change the belt on the engine is to do it with the pistons at mid stroke (NOT Top Dead Centre); specifically piston 1 (nearest the belt) on it's intake stroke. As described in the following paragraphs the camshafts are to locked at this position for the timing belt replacement procedure. The rattle is quite likelty the cam belt teeth coming off. Ignore the mechanic (top bloke) and 10:1 you will have a bill for new valves and a head overhaul. For goodness sake stop driving and get that belt changed.

Fiat aren't keen on timing marks, preferring tools to align/synch the camshaft/ crankshaft so you quite simply make your own marks before you take the belts off. It's not difficult.
if removing the lead with engine running makes no difference on that clyinder, and you know that the plug and lead are definately fine and sparkign, then the only possible problems are a head gasket leak on that cylinder (common on a 1.2) or an injector fault on that cylinder (very rare on the 1.2).
